• Qualifying order consists one Feliway or D.A.P. electric diffuser and vial, plus one additional refill OR three electric diffuser refills from a participating veterinarian.
• Valid only on purchases made from a veterinary clinic (internet purchases do not qualify).
• Submit the fully completed rebate form along with original receipt from veterinary clinic and UPC codes from the bottom of the product boxes.
